LAGOON DAY Permission Slip 2022
Date: LAGOON DAY will be Wednesday, May 25, 2022
In order to go with the school, students must:
- Maintain the appropriate behavior standards (i.e. no suspensions, etc.) over the next few weeks
- Paid all school fines
Regular, in person learning will be held for students ineligible to attend or who do not sign up to go to Lagoon.
- Cost: $50
- $2 bus fee payable to MJHS - must pay this by April 29 due to Lagoon’s deadline (if paid online through MySchoolFees, there is an additional $2 fee)
- $48 - park ticket; payable to Lagoon via online or at the park upon arrival. You are also able to use your Lagoon season pass for admittance. Please note that Lagoon will not take a check.
- Timing: We will be leaving MJH @ 8:00am (meet in front of the school) and buses will leave Lagoon @ 3pm! We plan to return to MJH around 4:30 pm. Note: If a student misses either bus then transportation is the parent’s responsibility.
- Bus Fee & Expectations: As stated above, there is a $2 bus fee payable to the school. If a student pays the bus fee, they must ride the same bus there and back from the park (No exceptions). In addition, every student will be pre-assigned a bus they are to ride on, it will be alphabetical order.
- Dress Code: Casual Dress. Please be aware that Lagoon nor the school is responsible for lost phones, hats, purses, etc. The Nebo School District dress code is expected.
- Behavior: ALL students are expected to behave in a respectful and responsible way. If any student is involved in behaviors that are illegal or inappropriate, then they are subject to the Lagoon authorities and will be detained at Lagoon until a parent can pick them up.
- Food / Lunches: If your student would like a pre-made sack lunch, indicate by marking the box on the permission slip below. If you do not pre-order a lunch, you are responsible to purchase your own food at the park.
- Riding with Parents: If a student decides to ride with a parent, then that parent becomes responsible for transportation to and from the park. In addition, if a student does not ride with the school there, they will need to pay the entrance fee at the park and NOT at the school.

Lagoon Bus PERMISSION SLIP--MJHS—Return/Pay $2 by April 29, 2022
I authorize my student ______________________________to go on the Lagoon trip, May 25, 2022. We agree with the above information and requirements. We will not hold Nebo School District or Mapleton Junior High School liable in case of accident or problems unless negligence on the schools part has occurred. I also recognize that my student will be expected to ride the same bus to and from Lagoon.
